1) New Version of HIPAA Research Authorization Form
- UCOP (UC Office of the President) has posted a revised English HIPAA Authorization for research on the UCOP website. The prior version (dated 2014) has been removed from their website and is no longer available.
- OHRPP is requiring the transition to this new form as of TODAY August 3rd, 2026, for newly obtained authorizations. Researchers will not be required to re-obtain authorization because of the new form.
- There are no non-English versions of the form available. As of now, it is the responsibility of the investigator to translate the form for any non-English speaking subjects.
- Because of the uncertainty of future translations by UCOP, OHRPP requests that any translated versions be sent to particpants@research.ucla.edu so that we can coordinate with UCOP to have this translated form available to other researchers on their website.
- A guidance document from UCOP is pending and will be announced in a future HRN when available.
- For any questions about the revised authorization, please contact the UCLA Health Office of Compliance Services.
